症状
Macのスクリーンショット機能（Shift+⌘+4+control）でスクリーンショットを撮影し、VSCodeの拡張機能であるPaste Imageを使って、Markdownに画像の挿入が出来ずに困っていた。
解決方法
最終的にキーバイントを変えて対応した。

⌘+Alt+V → control + ⌘ + a
に変更した
