I just tried reconnecting to a new org and saw this temporary org(valid for 5 days) populated for which I have reset my password and it worked! the dropdown showed the Initiate Return opton.
Adding Custom Actions Causes Copilot to Completely Failed
I'm essentially seeing the same result - the planner is calling the custom action but errors out.
A couple trouble shooting ideas:

I do believe the action likely needs to be on the opportunity object and not on the account object.

Another path to try is to remove the flow and see if that causes an issue (although that didn't resolve the issue in my SDO).
I have however had other custom actions work in copilot.
Lastly, one good troubleshooting tip is to see if the prompt template works directly on a field vs in copilot to help isolate the issue.
