I wish I had a better answer here, but my issue was resolved after disabling/enabling surveys in the 'Survey Settings'. I submitted a ticket with SF, but they weren't much help.

I think I figured out our issue, at least all of the failure examples I've seen from our service group are for contacts with related user records that are inactive. Still not 100% positive as to this though as surveys seem to be sending fine for contacts who don't have user records.
