To create data streams, there's one additional step: connecting your data source to the data cloud. First, go to the data cloud setup page. Under the "Other Connectors" section, create the S3 connection. Once this is done, the S3 connector will appear under the "Other Sources" section. From there, you can select the data source you want to connect.

I checked internally using the Gack ID you provided. It seems that if the CSV file contains any formula fields, it triggers the Gack error. Please verify if the CSV file contains any formula fields. If so, remove those fields and try again.

I was facing the same error. And I could notice that it was repeating the information in the Parent Directory field. So I opted to leave the next field blank. And I was able to move forward without any problems!
