確認した症状では受信者はPDFファイルを受けていない (添付がないメールを受ける)。
補足事項
- 新規メール送信では添付されない
- 返信メール送信では添付される
- Office 365アカウント
- 他の人から同じ宛先の新規メールでは添付される
- OS: Windows 8.1 Pro + Outlook 2013
- 受信側の問題ではなさそう

After doing some research on my own, I have found that the problem had to do with outlook attaching my signature and logo to my email before I attach the pdf. I don't know what the fix is other than disabling automatic signature and signing after attaching the pdf file. Hope this will be helpful to others.
署名機能を外して問題が出なくなったという例がある。
